Narrative for Morton Hospital

Pressure Ulcer Prevention
Morton Hospital’s healthcare team is dedicated to providing high quality care based on the latest evidence-based guidelines for the prevention and treatment of pressure ulcers. Risk assessments that focus on pressure ulcer prevention protocols are completed by nursing upon admission and every shift. Our skin team is represented by a nurse certified in both wound care (CFCN) and Ostomy Care (CWON), as well as select skin champions from the nursing units. Prevalence rounds are completed three times a year.

Fall Prevention
Morton Hospital has a dedicated Fall Prevention Committee and Administration that has made fall prevention and safe handling of patients to prevent injury a top priority. The staff is focused at change of shift safety huddles and post falls to provide the safest care for our patients. The Elderly Behavioral Health Unit has identified that their patients are at an inherent risk related to medications and aging, and are more vulnerable for injury. They’ve developed their own falls team and are implementing an action plan to ensure their patients are safe from injury. Both teams include Nursing, Pharmacy, Quality, Rehab Services, and Management, and we plan to enhance the team by including members from the Environmental and Transport Services staff. Both committees meet monthly.
